The applicant acknowledges and agrees to the need for verification of all information on this application. Applicant hereby authorizes all banks, businesses and persons identified on this application to disclose and furnish (both in writing and verbally), any and all information requested by Sancar Wallcoverings & Decorating, LLC, by telephone, fax, written correspondence, etc., and hereby releases, waives and disclaims any and all claims against Sancar Wallcoverings & Decorating, LLC, and all such banks, businesses, and persons arising out of such disclosures. If respresentations made by the buyer in this credit application are subsequently found to be incorrect or incomplete, the right is reserved to reject the application and to negate any obligation to proceed with any transaction.

I,residing at in consideration of credit being extended by Seller, Sancar Wallcoverings & Decorating, LLC, to the above named applicant for merchandise to be purchased, whether Applicant be an individual or individuals, a proprietorship, a partnership, a corporation, or other entity, hereby personally contact, guarantee and bind myself to the faithful performance of all obligations of Applicant including, but not limited to, payment on demand of any present or future indebtedness to Sancar Wallcoverings & Decorating, LLC by the Applicant whenever the Applicant shall fail to pay the same, whether secured or unsecured and regardless of how the indebtedness is represented or incurred and regardless of prior notice, demand or pursuit of remedies against the party primarily liable. 1 hereby consent to any extension or alteration of this guarantee without prior notice, and expressly waive all notice of acceptance, notice of extension of credit, presentment, and demand for payment on application, protest and notice to undersigned guarantor of dishonor or default by applicant or with respect to any security held by Sancar Wallcoverings & Decorating, LLC, extension of time of payment to applicant, acceptance of partial payment or partial compromise. The personal guarantee applies even in the event that the Applicant declares Bankruptcy or applies for Bankruptcy protection. I understand that this guarantee shall continue in effect until I have notified Sancar Wallcoverings & Decorating, LLC in writing via certified mail of its cancellation, but such cancellation shall not alter any obligation of mine arising thereunder prior to receipt of such written notice.

TERMS & CONDITIONS - MUST BE SIGNED TO COMPLETE APPLICATION

In consideration of the offer to extend credit, and for other good and valuable consideration received, or to be received by the Applicant, the Applicant, by the signature of its authorized representative set forth below, hereby agrees to the terms and conditions set forth in this document. DO NOT SIGN UNLESS YOU HAVE READ, AND AGREED TO, THE PROVISIONS OF THE CREDIT AGREEMENT SET FORTH BELOW.

(1) Buyer recognizes Seller's terms as Net 30 Days and acknowledges and authorizes a service charge of $30.00 plus 1.5% per month {18% annual) on any past due amounts. Returned checks will incur a service charge of $50.00. (2) In the event of default, Seller shall have the right to, (a) declare the entire amount due and payable, (b) limit the amount of credit extended or terminate buyer's account, (c) refer the account to collection. (3) Buyer agrees to pay all costs of collection, including any attorney fees which may be incurred. {4) At the option of Sancar Wallcoverings & Decorating, LLC, the venue for any suits, collection or otherwise will be in Kings County, New York, and buyer hereby submits to the jurisdiction of said courts and to the application of NYS law. (5) Buyer hereby represents and warrants that it is solvent and pays its obligations as they become due. This representation and warranty is deemed to be repeated in each purchase by buyer. (6) Faxed documents will be deemed as original. No oral agreements will be accepted. The terms on this credit application override all others.
